How a Geriatric Care Manager will service you:

• Provide comprehensive in-home assessment of the elder person's emotional, social, and financial needs.

• Establish an individualized action plan, after extensive consultation with caregivers, to address the unique issues of the elder person and begin implementation.

• Act as a liaison to family members at a distance.

• Monitor and modify services, as the elder person's needs change over time.

• Assist with alternate living arrangements, if it becomes necessary.
  

A licensed clinical social worker will provide a comprehensive in-home assessment of emotional, social and financial needs, establish an individualized action plan, coordinate and monitor resource services.

$80.00 per hour
