Services Provided
At
S.O.S., our focus is to prevent illness and injury and to maintain our
clients in their homes. The first step is the comprehensive assessment.
The assessment includes consultation with the client and their caregiver
system (family, neighbors, church members). Positive relationships are
fosters. Legal, medical and insurance issues are reviewed and discussed.
A Plan is developed and implemented.
The plans can be simple or complex depending on the living environment
and the client's physical and mental condition. Plans may include
reminders to take medications, pay bills coordinate MD visits and review
daily events. Care managers can accompany clients to MD visits is
requested. Complex plans may involve coordinating a move to a new living
environment or coordinating a sitter care and duties when transitioning
home from a hospital stay. These plans can be modified as the clients
condition warrants.
Brief therapeutic intervention, individual and family counseling
services are available.
